Sports Physicals
Many middle and high school sports programs require an Indiana High School Athletic Association (IHSAA) form completed and on file before participation in school sports.
The physical examination must be performed on or after April 1 by a physician holding an unlimited license to practice medicine, a nurse practitioner, or a physician assistant to be valid for the following school year.
- If your child has had a well-child checkup AFTER April 1, we can complete the IHSAA form with no appointment needed. Download the IHSAA form, complete the parent portion, and scan it to [email protected] or fax the form to 317-621-9190.
- If your child has had a well-child checkup before April 1 but within the last 12 months, we can schedule a sports physical (abbreviated well-child checkup) and complete the IHSAA form at that time.

NOTE: Scheduling your child's annual Well Child Checkup after April 1 will eliminate the need for coming in for two physical exams in one year.
